BSN@Home Scholarship Application

This scholarship application is for BSN@Home students for the Spring 2027 semester. Only current BSN@Home students and program applicants should complete this application. You are not eligible for School of Nursing funding if you are a new first year student at UW-Madison or if you are a pre-nursing student.   3. Describe your professional goals and your reasons for obtaining a BSN. (250 words or less)

  5. How many overall credits do you plan to enroll in at UW-Madison for the upcoming semester? Include both nursing and general education courses.
  6. Do you intend to practice nursing in Wisconsin after graduation?
  7. If you do intend to practice nursing in Wisconsin, would you be willing to sign a commitment letter agreeing to practice nursing in the State of Wisconsin for at least three (3) years of full-time service after graduation?
